Disclosure Forms

Relationships
If a vendor does business with or plans to do business with Sonora ISD, and is aware of a first degree relationship by blood (consanguinity) or marriage (affinity) with an Sonora ISD board member, the executive director, or a family member of either, that vendor is required to complete a Conflict of Interest Questionnaire. It is the responsibility of the vendor to obtain and submit the questionnaire to Sonora ISD. The form must be completed and filed by the vendor with Sonora ISD within seven business days of knowledge of the potential conflict.

In addition, the executive director or board member is required to file the Conflict of Interest Questionnaire if Sonora ISD is doing business with a vendor and a first degree relationship exists as described above. The form must be completed and filed with Sonora ISD within seven business days of knowledge of the potential conflict.
